Designed a custom Bill of Lading document type FE.

•This document uses essentially the same layout of the Type FE Invoice as a template except that the "Unit Price" and "Extended Amount" columns have been removed.

•The document name "Delivery Slip" will be printed immediately below the pre-printed invoice header and centered above the Bill To and Ship To addresses.
