Design a laser style version of this Customer Order Forms report per client specifications. The report will print the following columns: Pack Size; Item Description; Item Number; and Unit Price.  Blank columns will be printed for writing in the Order Qty and Lbs.  The report will be sorted by item class and include various outline boxes and shaded areas.
