Modify the layout of the Type A Invoice  to increase the width of the "Quantity Ordered" column.

Extended these changes to include setting aside an area at the bottom of the page for printing comments. (The original implementation of this invoice included setting this area aside but the "standard" layout for this invoice does not include this area.)
